The doctors welcome new patients who live within our practice areas. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.

There Are 3 Ways To Register With Us:

  • Online registration via NHS online
  • Online registration through our website
  • Download and print the forms at home, then bring them to the practice

Online Registration Through Our Website

Please complete the ‘New Patient Registration Form’.

If you choose to register online, please send via email two scanned copies of your ID. These should include at least one form of photo ID (ie. passport/driving licence whenever possible, and proof of address).

Please email your documents to: wyreccg.registrations.aylmer@nhs.net 

Alternatively, you can bring your documents into the surgery to be checked in person. Please note you won’t be fully registered until we see and approve your ID. We will need proof of name, date of birth and proof of address.

If you would like to opt out of having a Summary Care Record created please complete the relevant form, sign and date it, handing it in with the rest of your paperwork.

If you do not wish to register with us via NHS Online, or through our website, please download the forms, print them at home, and bring them into the practice with 2 forms of identification.


Temporary Patient Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
